The ice sits in your gutter, so the gutter looks like the problem. It almost never is — and that is why most fixes fail.
An ice dam is a ridge of ice at the roof edge that stops meltwater draining. It forms because of what is happening above the gutter, not inside it.
Warm air leaks from the living space into the attic and raises the temperature of the roof deck above freezing.
Snow on the warmed upper roof turns to water and runs down the slope toward the edge.
The overhang has no heated space beneath it, so it sits at outside temperature no matter what the rest of the roof is doing.
Meltwater reaching that cold edge freezes. Each cycle adds another layer, and the ridge grows until water backs up behind it and works under the shingles.
That said, gutters are not irrelevant. A gutter packed with autumn debris gives meltwater nowhere to go during a thaw, so water that could have drained instead sits and freezes. Clogged gutters do not start ice dams. They make the same weather produce a worse result, and they take the damage.
Almost all the damage attributed to ice dams is actually done by people removing them. Here is what each approach does.

Heated cables run along the roof edge and through the gutter to keep a drainage path open. They are the most commonly recommended permanent fix and the most commonly disputed one.
The case for them is straightforward: they keep meltwater moving, they cost relatively little for a spot installation at a known problem area, and they work on houses where fixing the attic properly is not realistic.
The case against is that they run on electricity all winter, they treat the symptom indefinitely rather than removing the cause, and there is a credible argument that keeping heat against roofing shortens its life. Some sources recommend them readily; others advise against them for exactly these reasons.
What both sides agree on is the underlying point. Cables manage the consequence of attic heat loss. They do not reduce it. If your roof is warming enough to melt snow from below, the durable fix is air sealing and insulation, and every source that sells neither says the same thing.
Prevention splits cleanly into work on the house and work on the gutters. Only one of those is a weekend job.
The house side, in order of effect. Air sealing first — gaps around light fittings, hatches, and pipe runs let warm air into the attic and matter more than insulation depth. Then insulation, where general guidance puts R-30 as a floor and R-38 as preferable. Then ventilation, so intake at the soffits and exhaust at the ridge keep the deck near outside temperature. This is the part that actually stops ice dams, and none of it is a gutter job.
The gutter side, which is genuinely yours. Clear the run completely before the first snow. A gutter full of wet leaves freezes into a solid block that holds every subsequent thaw against your fascia, and it cannot drain when the dam above it finally releases. Autumn clearing will not prevent an ice dam, but it decides whether the meltwater has somewhere to go when one forms.
